Healthy Youth Survey

The Healthy Youth Survey (HYS) is a statewide, anonymous, and voluntary survey distributed in class to Washington students in grades 6, 8, 10, and 12 that helps schools, public health agencies, and communities understand youth needs and improve programs. Middle schoolers will answer questions about their well-being, attitudes, behaviors, and experiences related to health and safety, providing data that helps guide public health policies, programs, and resources for all youth. 

Why It Matters:
Results help schools, communities, and public health agencies improve programs and services for youth across Washington.


Who Participates:
Students in Grades 6, 8, 10, and 12 statewide, and Grades 7, 9, and 11 in small districts.

School Supply

This Fall, our middle school students will participate in the Healthy Youth Survey (HYS), a statewide effort to understand and support youth well-being. The survey covers topics such as mental health, relationships, school experiences, substance use, and safety. Participation is voluntary, and students may skip questions or opt out entirely without penalty.
