Field Trip Permission Form

This form allows parents and guardians to give permission for their student(s) to attend a school field trip. Field trip details will be provided separately prior to submission of this form.

Please complete all required fields, including student names and grades, emergency contact information, and required acknowledgments. All checkboxes must be selected and the form digitally signed in order for permission to be granted.

Safety Acknowledgment - "I give permission for my child to participate in this field trip and understand that Wa He Lut Indian School will take reasonable precautions to ensure student safety during transportation and scheduled activities."

Liability / Hold Harmless Agreement - "I agree to hold Wa He Lut Indian School harmless from any liability arising from participation in this field trip, except in cases of negligence on the part of the school or its representatives."

Emergency Treatment - “I authorize school staff to seek emergency medical treatment for my child if I cannot be reached.”

Behavior & Expectations - “I understand my child is expected to follow all school rules and staff directions during the field trip.”

Transportation Acknowledgment - “I understand my child will be transported as outlined in the field trip details sent by the school.”
